Administrator, Belleview Valley Nursing Home Establish and direct overall operation of the facility and activities, internal and external, to assure excellent Resident care in compliance with established Deaconess Long Term Care policies, procedures and standards and federal and state regulations. Assist in the implantation of the facility State/Federal Plan of Correction. Review with the Corporate Consultant and/or Director of Clinical Services and Risk Management. Send a copy of the completed Plan of Correction to the Corporate Office. Operate the facility within budget for both labor and operations budget. Monitor and oversee the Overtime in each department. Provide monthly controllable financial reports to the Administrative Assistant and Regional Director of Operations in accordance with corporate standards. Direct the development and implementation of the Facility Marketing Plan that is submitted in writing to the Regional Director of Operations for approval. Market facility services in off-site, personal contact with community representatives and potential referral sources. Company Information: Belleview Valley Nursing Home is located in Belleview, MO. Phone: 816.509.8643 Email: kkraus@deaconessltc.org Social Services Director, Edgewood Manor, Raytown, MO The primary purpose of your position is to assist residents and families in making a smooth transition into the facility, provide customers with facility-related information via facility tours, personal visits/assessments, and manage the social services needs of Residents and families. Serve as a liaison between family, the resident, the facility and the referring facility in order to make a smooth transition into the facility. Schedule and conduct resident assessments relating to social aspect of resident care. Provide appropriate and timely documentation to the chart and Care Plans of assessments. Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the services. Develop a Care Plan (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified. Serve as a member of the Care Plan team, assisting in documentation of residents’ conditions and changes thereof. Provide adequate documentation for all services performed. Participate in discharge planning assuring appropriate documentation is available for a smooth transition out of the facility. Provide solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required. Interpret social, psychological, and emotional needs of the resident/family to the nursing staff, attending physician, and other resident care team members. Act as a case manager on behalf of the resident. Perform counseling as needed. Participate in community planning related to the interests of the facility and the services and needs of the resident and family. Requirements Two years’ working in health care community relations and marketing experience preferred. Experience working as a Social Services Designee or Director. Long Term Care experience preferred. Experience conducting professional presentations as exhibited in past experience. Expertise in working in a Windows and MS Office environment including MS Word and Excel and experience using networks and Email. Good organizational and planning skills as exhibited in past experience. Degree in Social Work or related field preferred. Certificate of Social Service Designee.
